If you’re managing a chronic health condition, you may be eligible for up to 5 rebated sessions per calendar year under a Chronic Disease Management Plan (CDMP) from your GP. Your GP will create a GP Management Plan or Team Care Arrangement, and refer you with an Allied Health Referral form, noting how many sessions are allocated to dietetics.
